Wouldn't It Be Nice... To have your own personal diabetes coach?
Let's face it. Very few people with diabetes get all the guidance they need from their physicians. Our multidisciplinary team of experienced Certified Diabetes Educators is here to coach you through the complexities of living with diabetes. We can help with the things that concern you most, such as controlling your blood sugars, diet & exercise and avoiding hypoglycemia and long-term complications.
To work with diabetes specialists who have diabetes?
Does your clinician understand what it’s really like to live with diabetes? When you work with Integrated Diabetes Services, you work with health professionals who either have diabetes or care for a family member with diabetes. We appreciate the work you put in and the frustrations you feel, because we deal with it every minute of every day!
To have your questions answered in a timely manner?
When it comes to your diabetes, no question is foolish, and no problem is too small. You deserve prompt answers to your questions. And if we don’t have the answer, we’ll search until we find it. You can access our staff by phone, fax or internet, and expect a response within hours – not days or weeks.
To receive guidance with the daily management of your blood sugars?
When it comes to diabetes care, nothing is more important (or complex) than blood glucose control. Our team can help you to design an insulin/medication program that suits your needs and your lifestyle. We’ll set up a record-keeping system just for you, and look for patterns and trends in your data. We’ll then provide ongoing analysis-based insight that only comes from years of focused attention on intensive blood glucose control.
To learn how to self-manage your diabetes?
There is a lot more to living with diabetes than just understanding the basics. Diabetes is a disease that requires continuous and extensive involvement and decision making by the person who lives with it. Our job is to give you tools, skills and knowledge to do an effective job of living with diabetes: From counting the carbs in a slice of pizza to making sure your labwork and examinations are done on time. We’re here to make you the expert on your own diabetes care.
To have access to the latest information and technology?
Too often, new tools and techniques for improving diabetes management take years to reach the people who need them. By staying on top of all the latest developments, we’ll keep you abreast of what’s coming and what’s currently available so that you can decide what is right for you. As a beta test center for many new diabetes care products, we can offer first-hand knowledge and insight into the newest innovations.
Integrated Diabetes Services provides a team of Certified Diabetes Educators with a passion for teaching diabetes self-management and helping you to achieve better control. We work to supplement (not replace) the usual services provided by your physician and other healthcare providers. We provide individualized consultation throughout the world to families and individuals of all ages via phone, fax, mail and live chat on the internet.
Step 1: The Assessment
Prior to your first consultation, we’ll send you a comprehensive written self-care assessment to complete and return to us. That way, we won’t waste your valuable time with endless questions and topics that are of no interest to you. By carefully reviewing your assessment form, we’ll learn as much as possible about you and your diabetes. Based on the assessment, your personal diabetes coach will begin formulating strategies and will forward materials that will be utilized during your initial consultation.
Step 2: The Initial Consultation
During the first consultation (via phone or live chat on the internet), we will: A. Discuss the information you provided in the assessment form and fill in any “gaps” that remain B. Review your current diabetes management plan and offer suggestions for better regulating your blood sugar levels. C. Develop a customized system for keeping records and communicating with our office. D. Provide essential/basic diabetes self-management education. E. Outline a plan for helping you to reach your diabetes management goals.
Step 3: The Follow-Through
At the conclusion of your initial consultation, we’ll discuss your follow-up options. Some people choose to take our advice and implement/fine-tune on their own. However, most choose to take advantage of our ongoing services. For those who do, three options exist:
1. A fee-for-service, whereby you can schedule consults with your diabetes coach and receive/pay only for the time and services you need. Each consult would include a review of your self-monitoring records, recommended adjustments, and a new educational topic in diabetes self-management.
2. A retainer service, whereby you pay for unlimited access to our services. This includes regularly scheduled appointments (usually monthly) and brief consultations between appointments for questions, review of self-monitoring records and self-management education.
3. A program package, for some of our more popular and widely-utilized services, including: a. Insulin pump regulation b. Diabetes management through pregnancy c. Carb counting & carb-based insulin dosing d. Weight Loss

Fees and Payment
Payment for initial consultations is due at the time of scheduling. Integrated Diabetes Services accepts Visa and Mastercard as well as personal checks and money orders. Upon receipt of payment, we will send you a billing statement (complete with the appropriate diagnosis and billing codes) for you to submit to your insurance carrier for reimbursement. Many insurance plans offer reimbursement benefits for diabetes management services. Check with your carrier or insurance administrator to determine whether you are eligible.
In order to minimize unproductive time and keep our services affordable and effective for all who want them, Integrated Diabetes Services does not bill insurance directly. We will always try to be flexible with those who are unable to afford our usual rates for consultation (see rate sheet below). Please speak with our office administrator to discuss alternative fees or payment arrangements.
Money-Back Guarantee
It is unlikely that you have ever come across a guarantee for health care services. After all, diabetes is hardly an exact science! However, we believe that you deserve to benefit from the time, money and energy that you invest in our services. Since starting our practice more than 10 years ago, our mission has been to improve the lives of people with diabetes. If we fail to do so, you deserve your money back. No questions asked. |
