What do you do when you have a successful private prac= tice serving 1500 patients in the greater Delaware Valley for more than 10 years?  You expand to a nation= al level, of course! 

 

There is a critical need for people throughout the cou= ntry to receive qualified one-on-one help with their diabetes self-management.  To quote the Jan. 31, 2005 edition= of US News & World Report, in a special section on the state of medical care, “…the average 17-minute office visit may not be sufficient (for physicians) to get enough information… and talk about the ever growing list of health issues they’re supposed to bring up.  That doesn’t even count time= to answer questions.”  Give= n the complexities of a disease like diabetes, most health care providers barely scratch the surface when it comes to teaching patients how to best manage t= heir condition.

 

People in all parts of the country are suffering needl= essly with poor control and complications. Our mission is to make a major differe= nce in the lives of people with diabetes across the country – just as we’ve done in the Delaware Valley.

 

We can now utilize phone, fax and the internet to h= elp people with their diabetes management all across the country.

 

Face-to-face meetings are not always necessary to help someone improve their diabetes

One thing we’ve learned from experience is that face-to-face meetings are not always necessary to lend assistance.  So we’ve set up the tools and means to offer our services remotely – via a toll-free phone number (877-SELF-MGT) and live chat capabilities at our web site (www.integrateddiabetes.com).  We’ve adapted our teaching materials, forms and educational literature so that they can be easily faxed, mailed, e-mailed prior to scheduled appointments.  An aggressive marketing plan is also in the works.

 

Those who utilize our “remote” (at-a-distance) services will have their very own “diabetes coach”, a Certified Diabetes Educator who has diabetes and a true passion for teaching & helping others. Individualized consultations are available via phone and live chat on the Internet for peo= ple of all ages with either Type-1 or Type-2 diabetes. 

 

After the initial consult= ation, follow-ups are offered as a simple fee-for-service, a r= etainer (which allows unlimited access to our services for a fixed period of time) = or a package program for popular services such as insulin pump regulation, diabetes management during pregnancy, carb= counting/insulin:carb ratios, and weight control.  A= s has always been the case, all clients of Integrated Diabetes Services receive ongoing technology updates and complimentary emergency supplies as needed/available.

DESA Conference May 12-14

 

The 2005 North American Conference of the Diabetes Exe= rcise & Sports Association (DESA) will be held at West Chester University in suburban Philadelphia.  Anyone= with diabetes as well as family members and health professionals are welcome to attend all or part of the part of the program, which features a variety of learning, networking and sports opportunities.

 

The program will focus on the unique interests and challenges faced by those with diabetes as they engage in various forms of physical activity.  Guest spea= kers will include national celebrities and local experts in the field of exercise and diabetes.

New Product Focus:          =

OptiClik (Lantus) Pen

 

For Type-1s on multiple injection therapy and Type-2s = on single-dose insulin therapy, Lantus has proven = to be the among the safest and most effective insulins available.  Now, taking Lantus has become easier than ever thanks to the new = OptiClik Pen from Aventis Pharmaceuticals.  

 

OptiClik uses 3ml (300 unit) cartridges of Lantus= insulin and standard pen needles (sold separately).  Doses are dialed in one-unit incre= ments from 1 to 80 units.  A digital= LCD display shows the current dosage to be delivered.&= nbsp; The dial can be turned backwards in case the dose is accidentally set too high.  Once the dose is se= t, a simple, smooth button push delivers the insulin.

IDS Walk Team Sets Fundraising Record

Sunday, October 31, 2004 was a day to remember – unseasonably warm, sunny and thoroughly beautiful – perfect for the Halloween edition of the JDRF Walk to Cure Diabetes.  Our Integrated Diabetes Services t= eam came out in full force – 83 walkers, including Spider Man (Benjamin <= span class=3DSpellE>Scheiner), Spider Man’s dad (who could that be?= ), and a friendly pack of four-legged supporters.=   Besides plenty of walkers in costume, there was a live band, face painting, arts & crafts, and lots to nosh on.  Best of all, we raised a team-reco= rd $14,000!  We’re proud to= have been part of the JDRF’s Greater Delaware = Valley walk sites which collectively raised $2.5 million to find a cure for diabetes.  Thanks to all who participated and contributed!

The Pump Room

 

A New Slant on Infusion Sets  = ;         

 

The number of infusion sets available for use with ins= ulin pumps has historically been limited to two primary types:  the straight-in sets (aka Sof-Set, Ultraflex, Quick-Set, Inset) and the angled sets (aka Comf= ort, Tender, Silhouette).  At Integ= rated Diabetes Services, our preference has always been the angled sets due to th= eir overall comfort, stability and resistance to kinking.  Now there is a new option in angled sets:  the Simple Choice= Easy and Simple Choice Easy Pro from Norcross, Georgia-based SpectRx.

 

The “Easy” and the “Pro” are identical except that the “Easy” features a universal Leur-Lock tubing connector and the “Pro” = comes equipped with tubing that fits the Medtronic/MiniMed Paradigm pumps.  Both can be inserted manually or with the Medtronic “Sil-Ser= ter” injector (there is an adapter that allows the Simple Choice to fit in the injector).  Simple Choice sets= come in your choice of 12mm or 17mm canula lengths (= the shorter version is preferred by very lean individuals), and with 23” = or 43” tubing.   They feature water-resistant tape to aid with adhesion, and a disconnect mechani= sm that minimizes the risk of accidentally dislodging the set.  A special pin at the connection si= te indicates which side of the tubing goes in face-up (to eliminate re-connect= ion problems).

The Paradigm 515/715          

 

Earlier this year, Medtronic/Mini= Med introduced its latest Paradigm insulin pump:  the 515/715.  Both pumps offer the same features= ; the 515 holds 176 units of insulin, while the 715 is slightly larger and holds = 300 units. 

 

Like the current 512/712 pumps, the 515/715 models spo= rt a compact, streamlined design and attachable clip; convenient AAA battery; di= rect radio communication with the BD Logic blood glucose meter; smooth-filling cartridges; a vibrate option; and an easy-to-use menu-driven format.  Programming features include a = 220;bolus wizard” calculator for helping to determine bolus doses, temporary ba= sal rates (set as a flat rate or a percentage of the usual rates), multiple bas= al patterns, optional test reminders, and the ability to bolus up to 10 differ= ent ways (utilizing a combination of “easy bolus”, “rapid bolus”, normal, square and dual-wave features).

 

Compared to the 512/712 pumps, a number of features ha= ve been added:

·        The home screen now features a running clock= as well as battery & reservoir remaining gauges. 

·        Within the bolus wizard, the duration of ins= ulin action can be customized to your preference. 

·        Users can set a target blood glucose “range” rather than just a specific number. 

·        When blood glucose levels are below 70, users can now bolus to cover a meal after activating through a warning message. 

·        The pump has a special mechanism for saving = and restoring user settings in case the memory is erased. 

·        The “daily totals” menu now prov= ides a variety of statistical averages regarding blood sugars, insulin usage and= carb intake.  <= /span>

Tuesday is Pump Night at DiabetesStation

Diabetes in Prime Time

 

The first national weekly TV program about diabetes ha= s hit the air.  Launched in March, 2= 005 on CNBC, dLife TV puts a human face on diabetes and empowers its viewers to take charge of themselves.  Segments include celebrity profile= s, medical news and innovation, exercise, nutrition, inspirational stories, fa= mily matters, kid’s corner, new products and advocacy.   The brainchild of Howard Ste= inberg (a Type-1 diabetic since age 10), dLife will pr= ovide an entertaining, informative and supportive program for the growing diabetes community.  Tune in to CNBC Su= nday evenings at 7 pm Eastern Standard Time, or log on to www.dlife.com.

 

 

Predicting Celiac Disease

 

Celiac disease is an autoimmune disorder that results = in intestinal distress following ingestion of gluten proteins found in many co= mmon grains (wheat, rye, barley, and possibly oats).  The prevalence of celiac disease in children and adolescents with type 1 diabetes is particularly high – = as much as 10% -- and growing.  A= new study conducted in Italy found that celiac disease is almost twice as commo= n in girls than boys, and more than three times more common when diabetes was diagnosed prior to age 4, compared to after age 9.  The presence of a thyroid disorder= was also linked to the development of celiac.&= nbsp; It is recommended that children with diabetes be screened for celiac disease, particularly those at high risk.&= nbsp; Speak to your doctor, or visit ww= w.celiac.org for more information.

 

 

New Neuropathy Drugs

 

The FDA has approved two new drugs for the treatment of diabetic peripheral neuropathy.  Pregabalin capsules (brand name = Lyrica, Pfizer Pharmaceuticals) are indicated for the management of nerve pain.  Duloxetine (brand name Cymbalta, Eli Lilly & Co= .) is intended for neuropathy management.  Lyrica works by blocking nerve cells tha= t are over-firing.  In clinical tria= ls, nearly 50% of those who tried the drug for 5-8 weeks experienced significant pain reduction.  Cymbalta was previously used as an antidepressant drug.  In controlled studies, more than h= alf of patients treated with Cymbalta experienced mild= pain reduction.  For more informati= on, consult your physician.

 

 

Financial Aid for Drugs

 

For those looking to obtain free or low-cost diabetes medications and supplies, two new resources are available.  The Cost Containment Research Inst= itute publishes a list of drug manufacturers with programs that offer drugs to pe= ople who cannot afford them, as well as a list of facilities that provide low-co= st health care.  Call (202) 318-0= 770, or visit www.institute-dc.org.=   Another web site, www.needymeds.co= m, provides up-to-date information ab= out 179 patient assistance programs run by drug manufacturers.

 

 

Deriving Your BG from Your A1c

 

Finally, policies have been set across the country to standardize HbA1c measurements, making the results equivalent between most labs.  To calculate your avera= ge plasma glucose level for the past 2-3 months, here is the magic formula: 

 

(A1c X 35) – 75. 

 

For example, If your HbA1c = is 7.6, multiply by 35 to get 266, then subtract 75 to get 191.  This is your 2-3 month average.   

 

 

Cholesterol and Eye Problems

 

Researchers at Harvard University analyzed data from m= ore than 1400 DCCT participants and discovered that those with high LDL cholest= erol (aka the “bad cholesterol”) were al= most twice as likely as those with low LDL to experience macular edema, the leading ca= use of vision loss in diabetics.  = Those with the highest ratios of total-to-HDL (“good”) cholesterol we= re almost four times as likely.  = The findings lend support to aggressive treatment of high LDL and total cholest= erol in patients with diabetes.

 

 

 


Parents Offer Tips for Lowering Kids’ A1c

 

At our recent Parents’ Support Group meeting, the attendees collaborated to develop a “Top-10” list of techniques= for reducing HbA1c in kids.  Since= A1c levels during childhood correlate with the develop= ment of complications later in life, this is a very important endeavor. 

 

The 15 parents spent several hours debating, prioritiz= ing and fine-tuning their recommendations, and came to a consensus on the list below.  The idea is not to sud= denly start doing every one of these at once, but to tackle one at a time.  Taking the advice of item 8, perha= ps you could ask your child which one he/she would like to try first!

 

Note:  We= tried to pare the list down to 10, but with no success.  So here is the Parents’ Top 12 Strategies for Achieving a Better A1c in Kids:

 

  1.   “Easing up” at times is OK, but always stay involved in your child’s diabetes care, and don’t ever give up.

 

  1.   Kids may not like stru= cture, but they need it.  Badly.=

 

  1.   Pumps or Lantus with mealtime shots generally work best.

 

  1.   Camps, conferences and local programs/fund raisers present great opportunities.

 

  1.   Insulin needs change, sometimes rapidly.  Adjust the doses when the rea= dings are not where you want them.

 

  1.   Teach them the art of comprom= ise.

 

  1.   Read, go on-line, attend programs, etc…

 

  1.   Laughter really is the best medicine.

 

 

Karpe DM Seeks a Speedy Remedy for Type-1<= /h1>

 

A new research consortium called “Karpe DM”, led by Dr. Claresa Levetan of Lankenau Hospital in Wynnewood, PA, is worki= ng to pass new therapies for Type-1 diabetes through the FDA within the next 24 months.  These therapies, which would eliminate the need to take insulin, involve a process known as Islet = Cell Regenaration.&= nbsp;

 

At a recent Type-1 Support Group Meeting held at Integ= rated Diabetes Services, Dr. Levetan and her team (including Zan Fleming, MD, an endocrinologist trained at NIH and experienced working at the FDA, and Loraine Upham, who was instrumental in helping Lilly develop = human insulin) explained a number of key concepts and steps in the process:

 

  1.   When in fact, islet cells die= and are replaced at a turnover rate of approximately 5000 islets per day in healthy people. 

 

  1.   The Gila Monsters (lizards) l= iving in the southwest eat only three times per year and make a hormone when they eat to regenerate their own pancreas. 

 

  1.   from ductal cells of the pancreas, = and from replication of existing islet cells.  Stem cells are not dir= ectly involved in the formation of new islets in humans.

 

  1.   The more well-known of these include INGAP, Exendin-4, GLP-1 and GIP.

 

  1.   They have a unique opportunity to = pursue Type-1 research through several pharmaceutical companies by privately fundi= ng the studies.  With sufficient private funding and a bit of luck, they hope to offer a revolutionary new treatment for Type-1 over the next few years.

    Dan Belisario

     

    On most fall & winter Sundays, in his top-row seat= at Lincoln Financial Field, braving the wind and cold, Dan Belisario can be found cheering on his NFC-Champion Philadealphi= a Eagles.  A season ticket holder since 1988 (the “Buddy Ryan” days, as he calls them), Dan is an avid fan of all the Philadelphia teams.

     

    When he’s not cheering on the Eagles, Flyers, Sixers or Phillies, Dan r= uns Zenith Marketing Enterprises, a business he started several years ago servi= ng as a manufacturers’ representative for a variety of building materials companies.

     

    “The business has me on the road most of the tim= e, at meetings, luncheons and conventions,” explains Dan.  “Being on a pump gives me the flexibility I need to do my job.  I’m not a slave to my insulin.  I tell everyone I meet who has dia= betes that they have to give the pump a try.”

     

    Dan’s first pump was a Mini= Med 506.  He has since transitione= d to a Disetronic H-Tron a= nd, most recently, the Deltec Cozmo= .  Not surprisingly, Dan’s moth= er (who has Type-1 diabetes) also wears a pump, as does his neighbor’s s= on at the Jersey Shore.
